On Wednesday,
December 29, 2011 at approximately 8:30 p.m., Officer Clifton Lewis, Star # 4103, assigned to the 015th District Tactical
Team, was fatally wounded during a robbery while working a second job at a convenience store in the 1200 block of N. Austin.
During the course of the robbery, Officer Lewis, age 41, acted in his official
capacity as a Chicago Police Department officer and was shot by one of the offenders and later succumbed to his injuries.
Officer Lewis joined the department on 27 April 03 and graduated in Nov 03. He leaves behind
a young daughter, a fiancée, a loving family and many close friends.

Police Officer Michael Flisk, 46, was fatally shot in the line of duty on November
26, 2010, while processing the crime scene from a burglary that had occurred sometime earlier.
Officer Flisk was an Evidence Technician assigned
to the E.T. Team - South for the past 3 ½ years. He joined the Chicago Police Department in January of 1991 and began his career in the 009th District. He also worked in the 004th District and Mass Transit
Unit before his promotion to Evidence Technician in 2007. He was a dedicated law enforcement professional and demonstrated
a commitment to serving and protecting the city of Chicago.
Officer Flisk received 67 awards during the course of his 19-year career. He earned
55 Honorable Mentions among many other notable achievements highlighting courageous and distinguished service.
Officer Flisk is survived
by his wife and four children. Three siblings are also members of the Chicago Police Department.
The Chicago Police Memorial Foundation would like to

Border Patrol Agent Michael Gallagher was killed in an automobile accident
on Federal Route 19 on the Tohono O'odham Reservation near Casa Grande, Arizona.
His truck was struck by a
drunk driver who ran a stop sign. The collision caused his truck to overturn, ejecting him from the vehicle. The drunk driver
was charged with voluntary manslaughter, DUI, and other charges.
Agent Gallagher had served with the United States
Border Patrol for two years and was assigned to the Tucson Sector. He is survived by his wife, two sons, mother, father, and
two sisters.

Michael Bailey.
On
18-JUL-2010, Police Officer Michael Bailey, Star #13970, was shot and killed as he was returning home from his tour of duty.
Officer Bailey was a decorated twenty-year veteran of the Department and was most recently assigned to the 001st District.
The investigation into his murder is currently ongoing.

The Chicago Police Memorial Foundation is saddened to report the loss of Officer
Thor Soderberg.
At approximately 3:45 yesterday afternoon, Chicago Police Officer Thor Soderberg, Star #14767, returned
to his vehicle at the end of his tour of duty. The vehicle was located in the parking lot of the police facility at
61st Street and Racine Avenue. While in that parking lot, the uniformed officer became involved in a struggle with a
24-year old male individual. During the struggle, the offender disarmed the officer and fatally shot him.
The
offender fled and is believed to have committed an armed robbery a short distance away. He fired additional shots, which
were heard by officers inside the police facility. Several officers responded and there was an exchange of gunfire.
The offender was shot during that exchange and was transported to a local hospital for treatment. His injury is considered
non-life threatening.
Soderberg, forty-three, was an 11-year veteran of the Department and was most recently
assigned to the Education and Training Division. He is survived by his wife.

The Chicago Police Memorial Foundation is saddened to report the loss of Officer
Thomas E. Wortham IV.
Officer Thomas E.
Wortham IV, Star # 6181, was murdered during an armed robbery attempt near his family's home in the 8400 Block of South
Martin Luther King Drive. Officer Wortham was able to return fire during the attack, but succumbed to his injuries and was
pronounced shortly after midnight on May 20th, 2010.
Officer Wortham joined the Chicago Police Department in June
2007 and served with distinction in the Englewood District. He also served our country as a First Lieutenant in the Army National
Guard and most recently completed a tour of duty in Iraq on March 30, 2010.
Two male offenders were shot during
the attack; one was pronounced dead and the other is currently hospitalized for his injuries. Two other offenders are in police
custody.
The
CPMF extends its deepest sympathies to the Wortham family and to the Chicago Police family. Please keep them in your

heavy gunfire early Monday, May 3, 2010 that killed one officer and wounded four others, authorities said. It was the first
time in five years that a Detroit police officer has been killed in the line of duty, Police Chief Warren Evans said.
DETROIT
(AP) -- Authorities say five Detroit police officers responding to a 911 call at a vacant home were met with heavy gunfire that
killed one officer and wounded four others.
Police Chief Warren Evans said several shots were fired as officers approached
the home about 3:30 a.m. Monday.
Police say two officers were treated at a local hospital and released and the two still
hospitalized are expected to recover. The suspect, a man in his mid-20s, was shot once in the lower back and was being treated
at the same hospital.
Evans said the officer who died was struck multiple times. Evans and Mayor Dave Bing visited with
that officer's widow at the hospital. The slain police officer had been on the job at least 12 years and has a 10-year-old
son.

Sgt. James Severin and Officer Anthony Rizzato were cut down by sniper fire while assigned to a "Walk and Talk"
program in the Cabrini Green housing development.
On Saturday 18 July 2009 at
10:am the 018 District will be hosting a memorial tribute commemorating the anniversary of the line of duty deaths
of Sgt. Severin and Officer Rizzato.
Included in the remembrance to these slain officers will be an unveiling in
front of the 018 District, of a permanent honorary street sign in their names by the surviving Severin and Rizzato
family members.
Help is needed to inform any involved present or former members, along with any interested
individuals of this memorial salute.
Please have interested individuals contact the 018 District community
